University Chapel & Galleries Reservation Form (External/Sponsored Group Use)

This form is to be completed after submission of a 25Live date and location request.

The use of the University Chapel is subject to the University Facilities Use Policy and the University Chapel Use Guidelines. External or sponsored groups may be subject to rental fees. All events in the University Chapel require the presence of Public Safety officers. To request,  contact: Waylon Miller, Associate Director of Public Safety, jmiller2@wlu.edu.
